<content:encoded><![CDATA[<p>When you have a Congenital Heart Defect, nearly every medical event in your life is affected by it. Even having your wisdom teeth pulled.</p>
<p>Before I go to see the dentist, he has to prescribe <a href="http://www.webmd.com/oral-health/antibiotics-before-treatment" target="_blank">antibiotics</a> to help prevent <a href="http://en.wikipedia.org/wiki/Endocarditis,_bacterial" target="_blank">Endocarditis</a>. Usually the drug is <a href="http://www.hsforum.com/stories/storyReader$1448" target="_blank">Amoxicillin</a>, since I am not allergic to Penicillin. An hour before my appointment I am to take SIX large capsules of the drug, and follow it with two capsule taken six hours afterward. I take my drugs exactly on time, because I have dealt with Endocarditis before. To say that it is not fun is like saying that a nuclear explosion could cause a slight disturbance.</p>
<p>So I pop my pills and go see my local dentist. The cleaning goes well, then in comes the dentist. He is masked and gloved up with a face shield. I remember the pre-AIDS days, when he would just come in, wash and dry his hands, plop down on the stool and get to work. That will probably never happen again.</p>
<p>Everything is going well when he suddenly says "Hmm...." I hate it when they say that that. "Hmm...." is doctor code for "Ut-oh."</p>
<p>"Problem?"</p>
<p>"Looks like you need to have these wisdom teeth removed."</p>
<p>Oh, great. I hate going to the dentist. Don't get me wrong, I have nothing against him personally, as far as I know he is a nice guy who loves his wife and coaches Little League. But his occupation has never impressed me and now he wants to start chopping.</p>
<p>"Ok, doc, can we do it today?" Let's get this over with.</p>
<p>"Not on your life. I'm sending you to an Oral Surgeon."</p>
<p>My dentist has always be very cautious about working on me, which I appreciate. But if I have to have this done, I want to get it done ASAP. So after a week of waiting and jaw pain (which very well might be imagined) I'm off to see the Oral Surgeon.</p>
<p>"Your local dentist has forwarded your records and we have spoken on the phone," he says. "Because of your heart, we'll do the procedure at the hospital and I'll admit you overnight."</p>
<p>Well, I'm not a fan of the hospital either, but if it has to be done, so be it. "How about anesthetic?" I ask. "You've got to be careful because I have low blood oxygen."</p>
<p>"I don't plan to knock you out," he says, "but we'll give you enough to keep the pain away. You'll probably doze off."</p>
<p>The wisdom tooth extraction took place in one of the operating rooms with everyone and everything fully sterile. True to his word, the Happy Juice the doc gave me had me quite drowsy. it also made me slap happy -- I told his nurse that she had beautiful blue eyes (she did!) and I'm pretty sure I even proposed to her!</p>
<p>As the surgeon predicted, I dozed off and woke up several times during the extraction, but I was too loopy to care about much of anything. Once I woke up to find the doc across the room, with someone holding a phone to his ear. Uh, doc... I'm over here! I thought. Deciding that I was just dreaming, I dozed off again.</p>
<p>I woke up with my mouth full of gauze and the doctor leaning over me, dressed in his street clothes. "Everything went great," the doctor said. "We'll keep you overnight and if there are no problems, you'll be released about noon tomorrow. I'm not going to speak to your parents because I have to go. Someone was <em><strong>kicked in the face by a cow</strong></em> and they need emergency surgery."</p>
<p>And I thought that I was having a bad day!</p>

<content:encoded><![CDATA[<p>Well, I was Ill all yesterday, and still am today, some lousy insignificant chest lurking antibiotic hating chest infection.  Got told to go to the doctors by work, and then got given antibiotics.  Lauren is understandably worried about how many antibiotics I take,which is not really that many, but I was Ill at Easter and Ill yesterday.  There seems to be a direct correlation between going home and getting ill, which is disconcerting to say the least, but either way, I am feeling much better now.  Felt rough this morning after a crap nights sleep, so did not go work on time.. went in an hour later, but since I did an hour yesterday morning before I was sent home, I figure it is neither here nor there.  So the amoxicillin seems to be working, and hopefully there is no chance of a super amoxicillin immune beast of a virus coming along and settling on my lungs, but on the off chance that there is, I will have to go to something more powerful!  So let's hope that does not happen!! Or that I am off amoxicillin now for a while, long enough to not create or foster a Mr Hyde virus.  NI just read that and it makes me sound like I am on drugs.  I assure you I have never taken anything that was not prescribed, and that is only my inhalers and amoxicillin normally.  They seem to like to dish it out to anybody with Asthma.</p>

<p>hal yang pertama dilakukan berobat ke dokter dan tebus resep. tau ga di saat bersamaan saya dan teman saya menghabiskan biaya berobat yang berbeda. saya Rp 30.000 dan teman Rp 200.000 setelah saya lihat kami diberi obat yang sama.</p>
<p>mau tau ga kenapa saya lebih murah? karena saya melakukan swamedikasi atau pengobatan diri sendiri. obat mahal belum tentu sembuh. yang benar pilih lah obat yang tepat sesuai dengan sakitnya. </p>
<p>oba untuk radang tenggorokan ada berapa ya? itu tergantung dari gejalanya. kalo gejalanya sama seperti yang saya tulis di atas, maka obat berikut bisa jadi referensi:</p>
<p>1. ANTIBIOTIK<br />
antibiotik yang umum yang bisa di gunakan Amoxicillin dan Cefadroxil. obat ini seperti panadol, aspirin, paracetamol generik. harga nya Rp 10.000-an sepapan diminum 3 kali sehari, jika panas sudah reda obat boleh di stop. obat ini gunanya sebagai pain killer mengurangi nyeri dan menurunkan panas.</p>
<p>3. MULTIVITAMIN<br />
kalo ini terserah pilih yang mahal atau murah. multi vitamin berisi vitamin C, B komplek intinya biar badan lebih cepat pulihnya. lihat tulisan saya tips memilih vitamin C.</p>
<p>Dengan pengetahuan umum untuk swamedikasi, mudah mudahan biaya kesehatan bisa di minimalkan. jika masih belum sembuh juga periksa ke dokter untuk pemeriksaan lebih lanjut. ingat bukan mahalnya obat yang membuat kita sembuh tapi pemilihan obat yang tepat.</p>

<p>Small talk makes me comfortable. When I'm nervous as hell, about to tear down some scabrous pathway with no clear end in sight, I talk. So I chat up Courtney. Colorful Courtney. Today she's wearing scrubs of a color nestled somewhere between the petals of a late-Summer sunflower and an orange sherbet Push Up. She's a dental assistant from the area who went to school in the same town where I spent my freshman year in high school. She was a toddler back then. She lives where I used to work . . . before I was laid off. She's sad to hear about that. She marvels at how small the world is. And she's probably tired of talking. But she keeps smiling – preparing all the tools and suction hoses and gauze pads and syringes and vial after vial after vial of Novocain, all the shade of urine and resembling those little glass tubes of free cologne I used to get at J. C. Penney – and never lets on.</p>
<p>Eventually Dr. E. comes in to numb up my lower jaw. He prods deep with his long Q-Tips and then goes for the nerves, moving the needle around, stopping to reload several times. I can't help thinking of Bill Cosby's bit about dentists. How they dig around as your mouth is gaping wide and try to engage in their own small talk. "Ever do any fishing?" I'm spared the humor; Dr. E. is all business. After a few minutes he leaves me to my Cubs game. I go numb while he is next door making impressions of another patient's smile.</p>
<p>So this is it. Yet another point of no return. I haven't let it hit me that way just yet but the thought is festering in the back of my mind, clawing its way to the front with an agitated determination that will defeat my denial eventually. It always does. Meanwhile the Cubs break it open in the 5<sup>th</sup> scoring six runs. It's exciting to watch. Even pitching phenom Carlos Zambrano gets a hit, a double to right that almost goes over the wall. I miss Harry Carey. I get bummed when I realize I'm watching a rebroadcast of last night's game. This is yesterday's excitement for millions and here I am cheering. The belated fanboy. Like the outcome of the game depends on my fist pumping and mumbled rally chants. Figures.</p>
<p>Dr. E. is back and he and Courtney have donned their masks, wheeled up their stools, tilted me back, pointed their strobe light and limbered up. <em>Let's do this!</em> I think to myself, a weak attempt at bravado, and I open my mouth and close my eyes. Find a happy place. He pokes. "Can you feel that?" I shake my head, very gently, side to side . . . Nope. "How about that." Nope. So he goes to work. I hear the scraping. Feel the pressure in my jaw but feel no pain. Even as he grabs a tooth and starts wrenching my head side to side. I strain my neck to keep my head buried on the sturdy blue pillow and try not to give in and move with each tug. I hear a tiny crack, like a lone, distant firecracker. My neglected tooth offers little resistance, giving easily to implements made of steel. A little suction, some gauze, a stitch. And then on the next tooth.</p>
<p>Twenty-six times.</p>
<p>Four hours.</p>
<p>A prescription for Vicodin and Amoxicillin and an instruction sheet basically telling me to get some rest.</p>
<p>I pause at the mirror mounted on the wall next to the receptionist's desk as I head out to meet my wife and youngest daughter. I smile weakly. My temporary dentures are already stained with blood and make my mouth feel way too small. I gag slightly at all the gauze. Then I slam my mouth shut, cover it all up with a cheap and reeking paper towel and head to the van for the unbearably long drive to Walgreens.</p>
<p>I catch a few glimpses of myself in the visor mirror as we drive. I'm at a loss for words. The sun has gone down, I'm freezing cold and I feel like shit. And I cry. Not gentle sobs but the kind of crying that rocks the van. The hopelessness that I imagined would come once I welcomed and walked past that point of no return is here, and it is brutal. I am not strong. I pity myself and hate myself and take no comfort in the fact that it's over. I let my anger and bitterness embrace me and shake me.</p>
<p>And eventually it passes. My pity party comes to an end when my daughter places her hand on my shoulder and tells me it'll be alright. Even as she betrays her fear and uncertainty with her pouty eyes, I sense a strength in her that is almost physical. I let it in and hug her, accepting her cute little offering of hope and fanning it with what small amount of my own I can muster. At the end of this day, even a little hope is a good thing.</p>

<content:encoded><![CDATA[<p>Welcome to Sunday.</p>
<p>I feel like crap today kids.  No, I'm not really sick.  No cold or anything either.  Just a headache, and oh yeah, it seems my body decided to turn into one giant 'measle'.  My head is a light shade of red and I have small red blotches all over the pasty white parts of my body.  It's not a pretty picture - so I'll spare you the rest of the visuals.  After a visit to the local Medi-center, it appears I've had a strong reaction to something.  Duh.  The Doc tried to pin it on the <a href="http://www.drugs.com/amoxicillin.html">Amoxicillin</a> I've been taking for the last week (thanks to my Dental peeps who are grasping at straws as to why a perfectly fine tooth decided to act up) but my skepticism is warranted ...I think.</p>
<p>I've had this 'allergy' before.  Not as wide spread as it is now, but the same symptoms and starting in the same areas.  I usually get something similar once a year - and it usually clears up within a few days before I can become alarmed about it.  Not this time though.  It started Friday night when I went to bed.  I noticed some redness on my arms and cheeks and kinda thought I might be allergic to dogs - as I'd just got home from visiting a quite friendly (albeit stupid) one.  But I doubt that's it.  It's not a contact reaction either ...if anything, heat seems to spur it on.  I found this out when I went for an hour long run (after visiting the Doc who simply prescribed <a href="http://en.wikipedia.org/wiki/Diphenhydramine">Benadryl</a>) hoping that I could flush the thing out with a good sweating and lots of water.  Nope - that only aggravated it, and the redness intensified and then travelled all over the place.  That was yesterday.</p>
<p>Ditto for a hot shower this morning.  The stupid 'dots' even spread out to my hands and feet after the 10 min. I spent under the tap!  Grrr.  I'm giving it the cold treatment tonight.  I got some plans to visit some friends in Vancouver this week, and I sincerely hope it calms down enough avoid the attention.  Stupid skin.  Stupid body!  I've got a full allergy test scheduled in a few months that'll hopefully reveal the problem.  Hopefully.  Maybe it'll reveal my seasonal allergies as well ...the ones I only started having within the last 5 or 6 years whenever springtime rolls around.</p>

<p>Poco male, bastano trenta secondi per cancellarli tutti.</p>
<p>Ieri sera però ho notato che il 99% di questi spam riguardano siti che vendono <em>"medicine"</em>, ovviamente senza chiedervi ricette o farvi troppe domande.</p>

<li><a href="http://en.wikipedia.org/wiki/Adderall">Adderall</a>, uno psicostimolante</li>

<title><![CDATA[Rosacea + Swelling?]]></title>

<content:encoded><![CDATA[<p>I saw a patient for a partner in clinic recently who has rosacea.  She has struggled with it for years, but it has gotten worse recently.  She has agreed to have a picture of her condition posted here for everyone to see:</p>

<p>Rosacea is a common skin condition.  The most frequent symptom is facial flushing, usually right over the cheek bones.  Also common are erythema (redness), sometimes with little red blood vessels visible under the skin and sometimes roughness also.  There's actually 4 types, but the biggest thing for most people is the redness.  Nobody knows what causes the problem.  We know genetics play a role, but it also seems to be brought on most commonly by things like: emotional stress, hot drinks, alcohol, spicy foods, exercise, cold or hot weather, and hot baths and showers.</p>
<p>The problem for this patient is that the areas are also swelling.  It's hard to see in the picture, but at times she has had swelling to the point that a friend wondered if maybe she had elephantiasis (lymphedema) of the face.  This patient has said that the swelling is as distressing as the bright red cheeks.</p>
<p>Historically, the pt. has taken amoxicillin for about 8-10 days to treat the problem and things have improved.  This isn't the most common antibiotic for rosacea, but it has worked for her.  However, she has had 3 outbreaks in the past month, all with bad swelling.  As soon as she stops the antibiotics, the problem comes back.</p>
<p>I've already seen this pt. together with a dermatologist, and he was a bit intrigued himself, so I'm not the only ingnoramous at this party.  Her primary care doc - who knows her best and has treated her outbreaks many times - is suspicious that she may have another problem entirely.  While seeing her, I took blood tests that argue against lupus, infection or other systemic inflammatory problems.</p>
<p>So, what does she have?</p>
<p>Don't know for certain.  Rashes are lame.  I never know what they are for sure.  The swelling is not a symptom that is commonly seen in association with the rosacea seen by some of the senior docs in our clinic.  However, at least one source I found says that the swelling can be even the ONLY symptom at presentation.</p>
<p>So, I'm going with rosacea.  Of the 4 types, I think she has Erythematotelangiectatic type.  I picked <i>that</i> one of the four because it's the longest and least-pronounceable and thus makes me appear smart and medicaly.  But is also seems to most closely fit her symptoms.  Why it is worsening is another question I can't answer.  I can only hope it gets better on our treatment plan.  Like many autoimmune conditions, rosacea doesn't really ever go away, we just keep it under control as much as possible.</p>
<p>And, sometimes, we give it really long sub-names too.</p>

<p>Can someone do the math here? How many people have been prescribed amoxicillin for this condition in the UK alone? How many years has this medication been on the market? Can someone do the math to see the extent of this problem? After how many years does this "study" show that something which may cause serious side effects has been used without even affect?<strong> </strong></p>
<p><strong>In an attempt to demonize homeopathy bad science has trivialized the deficiencies and even dangers of modern conventional medicine. </strong><em>This is not good for your health.</em></p>
<p>“In the British study, people suffering from facial pain and a runny nose with greenish or yellowish mucous generally improved within about two weeks -- whether they took the standard antibiotic amoxicillin, steroid nose spray or fake medicine. “</p>
<p>“Steroid sprays sometimes are used, but the study found they also were no better than dummy drugs, although they appeared to provide some relief for patients with only minor symptoms. “</p>
<p>Well, we’ve discussed the side effects of steroids on this blog but lets look at the side effects of amoxicillin:<br />

<blockquote><p>“Hypersensitivity Reactions<br />
Serum sickness–like reactions, erythematous maculopapular rashes, erythema multiforme, Stevens-Johnson syndrome, exfoliative dermatitis, toxic epidermal necrolysis, acute generalized exanthematous pustulosis, hypersensitivity vasculitis and urticaria have been reported.<br />
NOTE: These hypersensitivity reactions may be controlled with antihistamines and, if necessary, systemic corticosteroids. Whenever such reactions occur, amoxicillin should be discontinued unless, in the opinion of the physician, the condition being treated is life-threatening and amenable only to amoxicillin therapy.<br />
Liver<br />
A moderate rise in AST (SGOT) and/or ALT(SGPT) has been noted, but the significance of this finding is unknown. Hepatic dysfunction including cholestatic jaundice, hepatic cholestasis and acute cytolytic hepatitis have been reported.<br />
Renal<br />
Crystalluria has also been reported (see OVERDOSAGE).<br />
Hemic and Lymphatic Systems<br />
Anemia, including hemolytic anemia, thrombocytopenia, thrombocytopenic purpura, eosinophilia, leukopenia, and agranulocytosis have been reported during therapy with penicillins. These reactions are usually reversible on discontinuation of therapy and are believed to be hypersensitivity phenomena.<br />
Central Nervous System<br />
Reversible hyperactivity, agitation, anxiety, insomnia, confusion, convulsions, behavioral changes, and/or dizziness have been reported rarely.<br />
Miscellaneous<br />
Tooth discoloration (brown, yellow, or gray staining) has been rarely reported. Most reports occurred in pediatric patients. Discoloration was reduced or eliminated with brushing or dental cleaning in most cases.</p></blockquote>

<p>Sam went to the vet yesterday.  Since he was a kitten, he's always had an ear scratching problem, but on Sunday, we discovered he'd scratched a new area and it was large and freshly scabbed over.  So, off to the vet.  Diagnosis?  Allergies - could be diet, could be environmental.  I'm switching his food back to the one we know doesn't aggravate it, <a href="http://www.naturalbalanceinc.com/" target="_blank">Natural Balance Reduced Calorie.</a>  <a href="http://www.naturalbalanceinc.com/" target="_blank"><br />

<p>We adopted Sam at about 5 months old.  He came with the usual shelter health issues of ear mites and a respiratory infection.  The first vet we took him to, was an idiot.  And we were idiots for not recognizing it.  Sam was given some shots and then we were given amoxicillin<font face="Trebuchet MS, Arial, Helvetica"> </font>to squirt in his ears for ten days, and the vet showed us how.</p>
<p>Picture this:  one of us holding him, the other squirting 2cc of pink cold (refrigerated) liquid into our new kitten's ears and then massaging it down.  Then immediately throwing a towel over his head because he shook his head to clear his ears and pink stuff was flying...</p>
<p>After three days, he wasn't getting any better, his ears were caked pink, and he still scratched the hell out of them.  And he was still sneezing/wheezing.  At this point, you've probably recognized the error.  <strong><em>Um, when you take antibiotics, do you squirt it in your ears?</em></strong>  The next vet we took him to thought it was our misinterpretation of instructions, but I swear to you the first guy showed us how to squirt it in the ears and massage it.</p>
<p>So, antibiotics should be taken orally.  <em>Duh</em>.  After one day of squirting it into his mouth, his health improved. He started to play more and by ten days, he did all things kitty-like.  I can't believe that after that early ear torture, he let us ever pick him up again.  Either he has a short memory or a forgiving heart.</p>
